Requirements
To Participate in Athletics at Xavier College Prep High School all students must complete the following:

- Athletics Registration Card
- Physical Exam Card
- Authorization for Emergency Medical Care (Waiver) Card
- CIF Athletes Code of Ethics Form
- Weight Room (Waiver) Form
- School / Sports History Form (10th-12th graders only)
- Show proof of Medical Insurance or must purchase Myers-Stevens Ins. (see Colleen Mueller for information)
Please download all of these forms and return completed to athletic assistant, Colleen Mueller prior to participation in any athletics at Xavier. You don’t have to fill these forms out for each sport. You can fill them out at the beginning of the school year and be done with it for the remainder of that year. All these forms need to be filled out each school year.
